We're writing to inform you that this program will no longer accept samples after January 16th, 2026.

• Samples received on or before January 16th, 2026 will be processed and results will be provided as usual.
• Samples received after January 16th, 2026 will not be tested. If this occurs, PreventionGenetics will notify you to discuss possible next steps.

To order ALS genetic testing after this date, please:

• Consider the PreventionGenetics ALS Panel or ALS and Frontotemporal Dementia (FTD) Panel
• Contact your PreventionGenetics representative to discuss genetic testing options for ALS
